The Patient's Goals
For this 52-year-old international patient, the under-eye bags read as fatigue that sleep could not fix. The fullness came from herniated orbital fat rather than tiredness, sitting in puffy pads below a male lower lid. Surgeon's Expert Approach
For this male case, we chose a scar-sparing, fat-preserving approach because over-removal can hollow the lower lid and look unnatural.
- Transconjunctival fat repositioning: Working through an incision inside the lower lid, we repositioned most of the herniated fat into the tear trough and removed only the excess, leaving no external scar.
- nano-fat periorbital rejuvenation on hand: Where a residual groove remained, fine fat grafting was available to refine the tear trough and keep the under-eye contour soft.
- post-operative recovery suite: He completed recovery with HBOT and LLLT to reduce bruising and swelling and support a quick return to a rested look.
