How the Lower Eyelid Surgery Is Performed
Two routes are used, and the choice depends on skin quality rather than preference. When the problem is only herniated fat pads and the skin still has tone, the incision goes inside the eyelid (transconjunctival) and nothing is visible from the outside. When there is loose skin or fine crepey texture as well, a subciliary incision is placed just under the lash line and a conservative strip of skin is removed.
Fat is not simply excised. In most patients the pads are repositioned over the orbital rim to fill the tear trough, which is what removes the shadow that makes people look tired. Removing fat alone can hollow the lower lid over time, especially in patients over 50. Canthal support is added when the lower lid is lax, to protect the lid margin position during healing.
Operating time is 1–2 hours. Both general anesthesia and awake (local plus sedation) options are available; awake surgery is often chosen by patients who want to fly home sooner.

- Day 0–1: One night in hospital. Cold compresses every hour while awake. Mild pressure, not pain, is the usual complaint.
- Day 2–3: Bruising peaks. Vision may be slightly blurry from ointment. Short walks around the hotel are encouraged.
- Day 4–6: Swelling starts to drop noticeably. Sutures are removed around day 5–7 if a subciliary incision was used; transconjunctival cases have no sutures to remove.
- Day 7: Final check with the surgical team, then clearance to fly home. Most patients are presentable with tinted glasses.
- Day 10: Back to desk work and light routine. Concealer covers residual yellow bruising.
- Week 6–12: Scar line (if any) fades from pink to pale. Final contour of the lid–cheek junction settles.
Sleeping with the head elevated for the first week and avoiding heavy lifting for two weeks makes the biggest difference to how fast swelling resolves.

Who This Package Suits
The best outcomes come from patients whose main complaint is a visible bulge under the eye that stays the same regardless of sleep or salt intake. Good candidates also have reasonable lower lid tone; the lid should snap back quickly when gently pulled away from the eye.
Two situations need a different plan. If the darkness is pigment in the skin rather than a shadow, surgery will not change it. If the cheek has lost volume and the whole midface has descended, fat transfer or a midface lift addresses more than eyelid surgery alone.
Active thyroid eye disease, uncontrolled dry eye and recent LASIK within six months are reasons to postpone. Blood thinners are paused only under the direction of your own physician.
Why the Package Price Is Lower Than at Home
The $2,800 figure is the full package, not the surgeon’s fee with extras added later. In the US and Canada the surgeon’s fee, facility fee, anesthesia and follow-up are billed separately, and hotel or transfer costs are never part of the quote. In Istanbul the operating theatre, the anesthesia team and the hospital night are already inside the number you see above.

How long do I need to stay in Istanbul for lower eyelid surgery?
Seven days in total: one night in hospital and the rest at the hotel, with the final check before you fly. Booking a flight earlier than day 6 is not recommended because cabin pressure worsens swelling in the first days.
Will lower blepharoplasty remove my dark circles?
It removes the shadow caused by fat pad herniation and the hollow beneath it, which is what most people call dark circles. Pigment-based darkness in the skin itself does not change with surgery and is treated separately.
Are the scars visible?
With the transconjunctival route there is no external scar at all. With a subciliary incision the line sits under the lash line and typically becomes hard to see by three months.
Can lower eyelid surgery be combined with upper eyelid surgery in one trip?

Is under eye surgery in Turkey safe for international patients?
