They often happen when the incision is too short for the amount of skin removed, or when the side/flank tissues need more contouring. In many cases, they can be improved with a minor revision under local anaesthesia.

A tummy tuck is not only about removing loose abdominal skin. It is also about matching the shape of the incision to the way the skin, fat and flank tissue will close at the sides. That is why dog ears are best understood as a contour-planning issue, not simply as a “bad scar”. If you are researching how tummy tuck surgery is planned, this is one of the details worth asking about before you travel.

For UK patients comparing local private surgery with treatment abroad, dog ears can feel especially worrying because they raise a practical question: “What happens if I need a touch-up later?” This guide explains what they are, why they form, how surgeons try to prevent them, and what a correction usually involves.

Table of Contents

What “Dog Ears” After a Tummy Tuck Are?

Why Dog Ears Form?

How Surgeons Prevent Them?

How Dog Ears Are Fixed?

Revision & Realistic Expectations

Frequently Asked Questions: Dog Ears After Tummy Tuck

What “Dog Ears” After a Tummy Tuck Are?

Dog ears are small puckers or raised folds at the far left or right end of a tummy tuck incision. They sit where the main abdominal scar tapers into the hip or flank area. Some are tiny and flatten as swelling settles. Others remain visible because there is a genuine mismatch between the amount of skin removed in the centre and the amount of tissue left at the sides.

Medical image showing a scar-end fold at the outer edge of an abdominoplasty incision.

Skin folds at the outer ends of the incision

During a tummy tuck, the surgeon removes excess skin from the lower abdomen and closes the incision under careful tension. The central part of the abdomen is usually the area with the most loose skin. The sides are trickier. There may be less skin to remove, but more sideways laxity, thicker flank tissue, or a curve in the body shape that resists a perfectly flat closure.

When the incision ends are closed, that side tissue can bunch slightly. This creates a small raised fold at the scar end. It may look like a little point, tab or triangular fullness. From the front, it can interrupt the smooth line from waist to hip.

That does not always mean the tummy tuck has failed. A small dog ear can be a trade-off made to avoid extending the scar too far around the hip. A larger or persistent dog ear, however, may need revision if it bothers the patient or affects clothing fit.

Where the name comes from

The term “dog ear” is informal. It comes from the way the tissue fold can look like the folded corner of a page, or a small ear-like projection at the end of the scar. It is not a medical emergency by itself. It is a contour irregularity.

That distinction matters. A dog ear is usually about shape, not infection, bleeding or fluid build-up. If the area is soft, stable, not increasingly painful and not red or hot, it is often a cosmetic or contour issue rather than a medical complication.

Still, patients should not guess. If a swelling is enlarging, tender, warm, leaking fluid, or appears suddenly after activity, it should be assessed. In that situation, the issue may not be a dog ear at all. It may be swelling, a haematoma, or a post-operative fluid collection such as a seroma.

Dog-ear vs residual fat vs true loose skin

Not every bulge at the end of a tummy tuck scar is the same thing. A true dog ear is usually a fold of skin caused by how tissue closes at the incision edge. Residual fat is different: it feels fuller and more padded, often blending into the flank. True loose skin is different again: it may hang or wrinkle rather than sit as a small point at the scar end.

This is why dog-ear correction is not a one-size-fits-all fix. If the problem is excess skin, the answer is usually a small excision that extends or refines the end of the scar. If the problem is localised fat, careful liposuction may help. If both are present, the correction may involve both skin removal and fat contouring.

A good assessment looks at the tissue in standing position, not only lying flat. It also checks whether the contour issue is present on one side or both. Mild asymmetry is common after body-contouring surgery, especially during the first months of healing.

Why Dog Ears Form?

Dog ears form when the closure at the edge of an incision cannot lie completely flat against the surrounding body contour. The main causes are incision length, side-tissue laxity, flank fullness, BMI, and weight change. Sometimes the surgeon can predict the risk before surgery. Sometimes swelling hides the final contour until the tissues settle.

The incision is too short for the amount of skin removed

The most common reason is simple: the incision does not extend far enough to remove the side excess smoothly. Shorter scars are attractive to patients, and understandably so. No one wants a longer scar than necessary. But a scar that stops too early can leave a small standing cone of tissue at the edge.

This is the central trade-off in dog-ear prevention. A longer incision may create a smoother contour across the lower abdomen and hips. A shorter incision may keep the scar more limited, but it can leave side bunching. Neither choice is automatically wrong. The right choice depends on the patient’s anatomy and priorities.

This is especially relevant after pregnancy or major weight change, where looseness often continues around the sides rather than stopping neatly at the front of the abdomen. Details

Body shape, flank laxity and side-tissue tension

Dog ears are more likely when the abdomen and flanks do not have the same degree of looseness. For example, a patient may have significant lower abdominal skin but also thick side tissue that does not flatten with skin removal alone. When those tissues are pulled together, the side can pucker.

Flank laxity matters too. The flank is the area at the side of the waist and hip. If it has extra skin or soft tissue, a standard front-focused tummy tuck may improve the abdomen but leave side fullness. That side fullness can exaggerate the look of dog ears.

This is one reason some patients need additional contouring at the sides. The goal is not aggressive fat removal. It is smooth transition. Later in the article, we will touch on when liposuction can help with residual fat, without turning this into a full liposuction guide.

Higher BMI and weight fluctuation

Higher BMI can increase the chance of dog ears because there is often more soft tissue at the scar edges. That tissue does not always compress or flatten evenly. The thicker the side tissue, the more carefully the incision and closure need to be planned.

Weight fluctuation also matters. If weight drops significantly after surgery, skin that looked tight at first may relax. If weight increases, side fullness can return and make a previously small dog ear more noticeable. This does not mean patients must be at an “ideal” weight. It means results are more predictable when weight has been stable before surgery.

Healing also changes the picture. In the early months, swelling can make the scar ends look raised. Some apparent dog ears soften as inflammation settles. Others remain because they are structural. That is why most surgeons prefer to reassess the final shape after the main healing phase rather than revising too early.

How Surgeons Prevent Them?

Preventing dog ears starts before the first incision is made. The surgeon has to predict how the abdominal skin will move when it is pulled down, how the side tissues will close, and whether the flank needs to be addressed at the same time. For a sceptical patient, this is one of the most important consultation points: a smooth contour is planned, not improvised.

Surgeon consultation showing how incision length and lateral contour planning can help minimise dog-ear risk.

Incision-length planning

The incision length should match the real amount of excess skin. If the laxity is limited to the lower front abdomen, a shorter incision may be enough. If the looseness continues towards the hips, the scar may need to extend further sideways to remove the excess cleanly.

This is where honest planning matters. A surgeon who promises the shortest possible scar in every patient may be avoiding the more difficult conversation. A slightly longer scar, positioned low and planned carefully, can sometimes create a flatter and smoother result than a short scar that leaves puckering at both ends.

UK patients often ask whether this means the scar will be visible in underwear or swimwear. The answer depends on body shape and garment style. A well-planned tummy tuck scar is usually placed low enough to be hidden by most underwear, but the exact position should be discussed before surgery. It should not be left vague.

Flap transfer and lateral contour planning

A tummy tuck involves lifting the abdominal skin and fat layer, moving it downward, and closing it in a controlled way. The way this flap moves affects the scar ends. If the central abdomen is tightened without enough attention to the lateral tissues, the sides can bunch.

Lateral contour planning means looking beyond the front of the abdomen. The surgeon assesses the waist, flank and hip transition. If there is fullness at the side, gentle contouring may be needed so the scar ends do not sit next to a bulky edge. If the issue is loose skin rather than fat, liposuction alone will not solve it.

This is why a proper pre-operative assessment includes standing photographs, pinch testing, and discussion of weight stability. The body looks different standing than lying down. Dog ears are often most obvious when standing, turning, or wearing fitted clothing.

The honest trade-off: shorter scar vs smoother contour

Many patients understandably prefer the shortest possible scar. Surgeons prefer small scars too, but not at the cost of an avoidable bulge. In dog-ear prevention, the scar length and the side contour are linked. Reducing one too aggressively can compromise the other.

Surgeon-led perspective: A shorter scar is not automatically a better scar. In some body shapes, extending the incision slightly allows the side tissue to close flat. The honest goal is not the shortest line on paper; it is the smoothest contour that will still look natural once swelling settles.

This does not mean every small dog ear must be prevented with a long scar. Some tiny edge fullness settles. Some patients would rather accept a minor touch-up later than extend the original incision. The key is that the patient should understand the trade-off before surgery, not discover it afterwards.

UK planning note: If you are comparing consultations, ask your surgeon how they decide where the scar ends. A GMC-registered surgeon in the UK, or a European Board-Certified team working to GMC-equivalent standards abroad, should be able to explain the incision plan, the dog-ear risk, and the revision policy clearly.

<div class="jet-listing-grid--blocks"data-element-id="08bbab22-1366-43b2-a8bf-0c765fd2374b"data-listing-type="blocks"data-is-block="jet-engine/listing-grid">

.elementor-12947 .elementor-element.elementor-element-c15d156{--display:flex;--flex-direction:row;--container-widget-width:calc( ( 1 - The method depends on what is causing the bulge: extra skin, residual fat, or both. Most corrections are considered only after the tissues have softened and swelling has settled enough to show the true final contour.

Minor in-office excision under local anaesthesia

If the dog ear is mainly extra skin, the usual correction is a small excision. The surgeon removes the raised fold and extends or refines the end of the scar so the tissue lies flatter. This is often done under local anaesthesia, meaning the area is numbed while the patient remains awake.

The correction is usually shorter and easier than the original abdominoplasty. There is no major abdominal tightening, no muscle repair, and no large skin flap. The incision is limited to the dog-ear area. Recovery is therefore typically quicker, although the exact downtime depends on the size of the correction and whether liposuction is added.

A peer-reviewed discussion of dog-ear correction describes the importance of addressing the side fullness and flank laxity rather than simply trimming the surface fold. For patients, the practical lesson is simple: the revision should correct the cause of the dog ear, not just make the visible point smaller. A peer-reviewed technique for correcting dog ears explains this contour-based approach in surgical terms.

Liposuction for residual side fat

If the dog ear is partly caused by residual fat at the sides, a small amount of liposuction may be used. This is not the same as performing a full liposuction procedure. It is targeted contouring to smooth the transition between the scar end and the flank.

Liposuction works only on fat. It does not remove loose skin. If the bulge is mainly a skin fold, liposuction alone can make the area look looser. That is why the surgeon has to decide whether the correction should be skin excision, liposuction, or a combination.

For patients who want to understand the fat-removal side of this decision, it may help to read about how liposuction removes residual fat. In this article, the important point is boundary control: dog-ear correction is usually a small refinement, not a substitute for a properly planned tummy tuck.

Why surgeons usually wait until healing settles

Dog ears can look worse in the early healing period because swelling gathers around the scar ends. The tissue may feel firm, raised or uneven. In many patients, this improves naturally as inflammation reduces and the scar matures.

For that reason, surgeons usually avoid revising dog ears too early unless there is a specific medical reason. Revising during heavy swelling can lead to overcorrection or an unnecessarily long scar. Waiting gives the surgeon a clearer view of what is temporary and what is structural.

The timing varies, but many surgeons reassess several months after surgery, once the main swelling has settled. The broader swelling pattern is covered separately in how swelling settles over time, because dog ears should not be judged in isolation during the early recovery phase.

Issue

Typical approach

When it is considered

Anaesthesia

Recovery expectation

Small skin-only dog ear

Minor excision of excess skin

After swelling settles and the fold remains

Usually local anaesthesia

Shorter recovery than the original tummy tuck

Dog ear with residual side fat

Small excision plus targeted liposuction

When the flank edge is bulky as well as folded

Local anaesthesia or light sedation depending on extent

More swelling than skin-only correction, but still limited

Wide side laxity or poor original contour

Larger scar revision or extended contour correction

When the issue is broader than a small scar-end fold

Depends on surgical plan

More similar to a revision procedure than a touch-up

Dog-ear correction should be planned with the final scar in mind. Removing the fold may slightly lengthen the scar. For many patients, that is a worthwhile exchange: a longer but flatter scar can be easier to hide than a shorter scar with a visible raised end.

ASPS guidance on tummy tuck scarring also notes that a tummy tuck scar can take time to fade and mature. That broader scar timeline is relevant here because a dog-ear revision creates a new, smaller scar that also needs time to settle. What to expect from a tummy tuck scar gives a useful patient-facing overview of scar maturation.

Comparison image showing when a small dog-ear correction may differ from a larger planned abdominoplasty revision.

Revision & Realistic Expectations

Dog-ear revision is usually a refinement, not a sign that the whole tummy tuck needs to be repeated. The key is deciding whether the problem is small and localised or part of a wider contour issue. A minor dog ear can often be corrected simply. A broader side bulge may need more planning, especially if there is residual flank fat, loose skin, or uneven healing.

Minor revision vs full tummy tuck revision

A minor dog-ear revision focuses only on the end of the scar. It may involve removing a small triangle or ellipse of skin so the incision closes flatter. This can slightly extend the scar, but the trade-off is usually a smoother outline under clothing.

A full tummy tuck revision is different. That may be needed if the original result has wider concerns: loose abdominal skin, poor scar position, irregular contour, muscle repair issues, or significant asymmetry. Dog ears alone do not usually require this level of surgery.

This is why language matters. Calling every dog ear a “failed tummy tuck” is not accurate. Some are minor edge issues. Some are preventable planning issues. Some reflect the patient’s anatomy and the chosen balance between scar length and contour.

Healing after a dog-ear correction

Healing after a small dog-ear correction is usually more limited than the first operation. There may be local swelling, bruising, tightness and a short new scar at the edge of the old incision. Most patients find it easier than the original tummy tuck because the correction is much smaller.

The new scar still needs time. It may look pink, firm or slightly raised at first before flattening. Scar maturation can take months, so the result should not be judged immediately. Sun protection, scar care and avoiding tension on the area all help the scar settle more predictably.

Dog-ear correction also needs realistic expectations. The aim is improvement, not mathematical symmetry. Human bodies are not perfectly mirrored. A good correction should make the scar end flatter, less visible in clothing, and more in line with the surrounding hip and waist contour.

Can dog ears come back?

Dog ears can come back, but it is not common when the cause has been properly addressed. Recurrence is more likely if weight changes significantly, if the original side laxity was underestimated, or if the correction removes too little tissue to avoid extending the scar.

Patients can reduce the risk by waiting until the result is mature before revision, keeping weight stable, and choosing a surgeon who explains the scar-length trade-off clearly. Revision should be timed when swelling has settled enough for accurate planning.

Cost and result photos are important, but they should not dominate this article’s scope. For pricing, use the dedicated page for the cost of a tummy tuck in Turkey. For visual context, review healed tummy tuck results rather than judging your own scar-end contour too early.

It is also worth keeping dog ears separate from other recovery concerns. A raised fold at the scar edge is different from belly button healing, which has its own normal stages and warning signs. If your main concern is the navel rather than the scar end, read about how the belly button heals after a tummy tuck.

AKM perspective: For international patients, dog-ear prevention starts with careful pre-operative photo assessment and is confirmed again during the in-person surgical plan in Istanbul. The aim is to make the lateral scar ends as smooth as the central abdomen, while being honest about when a slightly longer incision gives a better contour.

Frequently Asked Questions: Dog Ears After Tummy Tuck

These FAQs answer the common UK patient concerns around dog ears after abdominoplasty: whether they are normal, whether they need correction, and how to tell them apart from more urgent post-operative problems.

Are dog ears a surgical mistake?

Not always. Dog ears can happen because of anatomy, flank laxity, BMI, swelling, or the decision to keep the scar shorter. A large or avoidable dog ear may reflect poor incision planning, but a small edge fold is not automatically a surgical mistake.

Will dog ears go away on their own?

Some small dog ears improve as swelling settles and the scar softens. True skin excess is less likely to disappear completely on its own. If the fold is still visible after the main healing phase, a minor revision may be considered.

When can dog ears be fixed?

Most surgeons wait until swelling has settled and the final contour is clearer. This is often several months after surgery, though timing varies. Revising too early can make the scar longer than necessary or correct tissue that would have settled naturally.

Can dog ears come back?

They can, especially after significant weight change or if the original side-tissue laxity was not fully corrected. A seroma is a fluid pocket that may feel soft, wavy, or swollen and can appear after activity. If swelling is enlarging, painful, warm, red, or leaking fluid, contact your surgeon promptly rather than assuming it is a dog ear.

Does the NHS or insurance cover revision?

Dog-ear revision after cosmetic tummy tuck surgery is usually considered cosmetic unless there is a clear medical problem. UK coverage depends on individual policy terms and clinical circumstances. Patients should check this before surgery and ask the surgeon what is included in their revision or aftercare policy.

Can dog ears be prevented completely?

No surgeon can guarantee that dog ears will never happen.
